We’re dedicated to making it easy to be a patient at Carolina Bone & Joint.  Please note these steps for paving the way for smoother visits, payment and follow-up:

Print, complete and fax the following forms prior to your appointment, or bring them with you to your appointment.  Fax numbers for each office are located under “Contacts & Locations” on this website.

Before your visit: We accept most insurance plans and worker’s compensation cases. Please contact your insurance company to verify our participation with your plan, and  bring your insurance card to each office visit.  If you have questions about insurance authorization, call 704-541-3055, and select the billing option.  If you do not have insurance, please come prepared to pay in full at the time of service.

After your visit: If you submit disability forms, allow two weeks for processing.  A processing fee and patient questionnaire are required prior to submission of each form.

To obtain medical records, call 704-541-3055 and select the medical records option.  You will be asked to sign a HIPAA release.

Call your pharmacy to request prescription refills, allowing ample time for processing them.
